By proceeding with a treatment you have agreed the service being performed and these terms and conditions. You acknowledge that due to: Application over another product; application of another product by yourself or others; the prolonged use of “stick-on” nails; the medical condition that you have; the fact that you are a nail biter, prior damage to your nails, the results of your treatment may be compromised through no fault of Pampered & Polished. You acknowledge you may have complications for which you do not hold Pampered & Polished responsible. You acknowledge that these Enhancements should be infilled (rebalanced) every 2-3 weeks or professionally removed at a reputable Salon. These Enhancements/treatments are not guaranteed and are your sole responsibility once your appointment is completed. Any breakages or repairs must be paid for. You are not knowingly suffering from any transferable disease or infection. You must inform the salon if any of the consultation details you have given change in the future. You are over 16 years of age or 15 years of age with a parent or guardian present and providing written consent for you to have a treatment. You agree to receive promotional material (offers, vouchers, notifications of special offers etc) electronically or through the post. You consent to your information being stored safely by Pampered & Polished, with the understanding that it shall be subject to guidelines set out in the Data Protection Act. You accept that appointments are subject to a lateness, cancellation and missed appointment policy, (Appointments are subject to a non-refundable deposit of £5. Clients who are not present for their allotted appointment time may not be able to have the treatment that they booked in for, and if an appointment is cancelled less than 24 hours before the appointment. time or a client does not show up for a booked appointment, they will be required to pay a non-refundable deposit of 50% of treatment cost for their following appointments) You accept that you need to give a non-refundable deposit of 50% of the treatment cost per treatment booked for appointments booked during the Christmas/New Year Period.

2018 Pampered & Polished Nails Birmingham
Under the new GDPR coming into force at the end of May 2018, I am required by law to document what personal information I keep for all clients, whether they be a one off appointment or regular clients.
At the time of your appointment I will take your name, address, a contact telephone number and an email address with your consent and also any medical information/history which you feel is relevant and that may have an impact on the suitability of nail enhancements.
All previous and future clients have a treatment card which records all the information above plus the treatment you are receiving, the date of that treatment and your signature. These records cards are kept electronically, secured using fingerprint recognition which only I have access to. Cloud storage which backs this up is GDPR compliant. I have some previous clients details stored as a physical paper copy; these are in a locked secured place which only I have access to.
I am required to keep this information for legal and insurance purposes. All information is kept for a period of 7 years from your initial communication after which it will be securely destroyed unless you are still a client of mine. You do have the right to see any information I keep and you can request that the data be changed or securely destroyed. If however you do request that records be destroyed I will no longer be able to carry out any treatments for you.
I do not use the information for anything other than to contact you regarding your appointment or to discuss matters relating to your nails.
I will not pass your personal information on to any third party.
I will not use your personal information for marketing purposes without your consent.
Any information given to me via the message service on my Facebook page or Instagram page is only accessible to myself, Gemma Rose. You may request that once I have written your details on your record card that I can delete any messages which contains your personal information.
Any information given to me via text message on my mobile phone is accessible only by myself and my phone is locked with a security code and fingerprint recognition. You may request that any text messages are deleted which contains your personal information.
